We accept new patients by referral from another medical provider. After we receive the referral, we will reach out to you to schedule an appointment.
We have a set of intake forms necessary in advance of your first appointment with us. These are completed online at eCheck-In prior to your visit.
Please bring your current medication list (or pill bottles), photo ID, insurance cards and co-payment if applicable. We may ask for a urine sample at your first visit. If you did not submit your new patient forms through eCheckin and were given a paper packet instead, please bring this with you as well.

You can expect a discussion with your provider about your kidney health and possibly a physical exam. If it's your first time in the office we may ask for a urine sample. If so, we will provide instructions and supplies to obtain a sample during your visit.
The New Patient Packet must be completed for us to evaluate and treat you. If you did not complete this ahead of time, either on paper or through eCheckin, please arrive to your appointment 30 minutes early and we will provide a paper copy to complete before your visit.
Yes, we have a "no show" and late cancellation policy. If you are unable to keep your scheduled appointment please notify us at least 24 hours in advance so we can accommodate other patients. Multiple late cancellations (less than 24 hours) and "no shows" may not be rescheduled.
No, we do not operate a full-service laboratory. If we recommend additional labwork, you will be given a lab order to take with you to the laboratory of your choosing. We typically request labwork for completion 1-2 weeks prior to your next visit. All of our offices are located near a hospital with an outpatient lab, and there are several independent laboratories around Central Ohio.
If you need to fast before your labwork, this will be displayed on your lab order form. If unsure, feel free to call our office for clarification.
If you need another copy of the lab order please call our office as there are many ways we can get this to you. If you're going to the lab within a couple days, we can even send it to the lab directly if you tell us where to send it.
Although we review labwork from other providers, if you were given a lab order from our office it is likely nephrology-specific. If you are needing to complete labwork for another provider at the same time as ours, simply take both lab orders to the lab as they are often able to complete both orders at once. If you are unable to obtain your labwork please call our office for guidance.
If you were recommended to have diagnostic testing such as imaging studies, we will send the order to the facility of your preference. Typically the facility will arrange scheduling but sometimes our office will call you, depending on the facility policy. If you have not heard from anyone in a week, please call our office.
